Child/Youth Registration

Consent and Authorization

I/we give permission for my child(ren)/youth to participate in the activities of St. Paul Lutheran Church, on the church premises. In consideration of the opportunity of my/our child/youth to participate in the activites of St. Paul, I/we release St. Paul Lutheran Church, its officers, agents, employees, staff, and volunteers from any and all liability of any kind whatsoever for any loss or injury to my/our child/youth arising from my/our child/youth's participation in the activities of St. Paul; and I/we agree to indemnify and hold forever harmless the St. Paul Lutheran Church, its officers, agents, employees, staff, and volunteers from any and all liability of any kind whatsoever for loss or injury to my/our child/youth arising from activites on the premises of St. Paul Lutheran, including loss or injury resulting from negligence or gross negligence.

I/we understand that my child(ren) may be photographed in the activities of St. Paul Lutheran Church. Recognizable images of my child may be used to post on the church, Facebook page, or bulletin boards. I understand that a non-recognizable images, such as a group picture, may be posted without explicit permission.

In the event of an emergency, I authorize St. Paul Lutheran Church representatives to seek medical treatment for my child if I cannot be reached. I agree to assume responsibility for all medical expenses incurred. I release them from liability in accordance with Florida Statue 1006.062.

Behavior Expectations

To ensure a safe and positive experience for everyone, all youth are expected to:

• Follow directions given by leaders and chaperones.

• Show respect to others, property, and themselves.

• Use appropriate language and behavior at all times.

• Refrain from the use of tobacco, alcohol, vaping, illegal drugs, or inappropriate media.

• Refrain from sharing, giving, selling, or offering any medication to another person under any circumstances.

• Use medication only as directed and only by the student for whom it was prescribed.

• Stay with the group and follow the planned schedule.

• Keep cell phone use limited to appropriate times as directed by leaders.

• Participate fully and maintain a cooperative attitude.
